Students who demonstrate a disability that substantially limits one or more of life’s major activities and requires reasonable accommodations may be eligible for a 504 Plan. A request for referral for a 504 evaluation to determine eligibility should be forwarded to Principals at the Elementary Schools, the Guidance department at Thurston Middle School and the Guidance department at Westwood High School.
